Personal Information Questionnaire

Personal Information Questionnaire, as it's name suggests is a form about a candidate's personal information. This form is filled by candidates on their arrival at Service Selection Board; while some boards get this form filled by only screened in candidates;i.e.candidates retained after stage-I testing.
Candidates often think what is the purpose of this form? Candidates who have previously been at SSB,must be knowing it's importance as the questions asked in personal interview are based on this questionnaire. So, never try to bluff or exaggerate about yourself in PIQ. Write correct information in this form as if the information given in this form contradict with what you speak during interview can put you in trouble.
It's important to know that the PIQ form has a major role in your selection as you have to fill two copies which are given to interviewing officer as well as to GTO.

In the starting of this form, you've to fill in the basic details,i.e., your name, your father's name, place of maximum residence & your present residing place, along with the approximate population of that place(or city). Thus it's important to know these things well in advance & better than guessing it.
After that, you're supposed to write your state & district, religion, your category, mother tongue, date of birth, whether your parents are alive or not,if not, then your age at the time of father's/mother's death.
After that you have to give details of education, occupation and monthly income of your parents/guardians and siblings.Hence before going to SSB, you must know these.
After that you have to write your educational record starting from matriculation-school/college,board/university,passing year, percentage marks, medium of instructions, whether you were day scholar or a boarder, followed by outstanding achievements.
On the second page of this form, you have to write your age(in months & years), your height & weight.Hence you're advised to know your height and weight(even approximate, but never write you don't know).
Next thing you are asked is your present occupation and present monthly income(if you are serving).
Next thing asked is whether you have taken N.C.C. training or not.If yes,you are supposed to give details of total training, the wing and division, and the certificate you obtained.
Then you have to mention your participation in sports and games, including the game you played, level, period of participation and whether you achieved something outstanding.
Next you are supposed to write your hobbies/interests,your participation in extra-curricular activities along with details of the activities;followed by any position of responsibility that you have taken(in sports,N.C.C.,or scouting etc.).
Next, you have to write nature of commission-permanent or short service commission, choice of service. Next is for repeaters, you're asked whether you have taken chance for commission previously or not. Freshers will write no and leave the next one blank(or write-not applicable).
Next, the repeaters have to give details of all their previous attempts-type of entry, the board, date and place, batch no. and your chest no. There's no issue if you don't remember the batch no or the ssb no exactly. Write whatever you remember. But don't ever try to hide your previous attempts if you were not recommended/screened out.
These are all the details you have to fill in your PIQ form.
As PIQ decides most part of your interview, don't bluff, and don't write a responsibility,or a game,or a hobby which you haven't done.Don't try to bluff at all.
Besides these, as this form makes your first impression on the interviewing officer, because he reads it before interviewing you. Hence you're suggested to fill it in a good, clear & legible handwriting avoiding any mistake, cutting, overwriting or using whitener etc.
I hope this article will help you understand the PIQ form,as well as to know its importance and the write way to fill it. Your suggestions and queries are welcome. Do mail your any queries at dashinganujdude@gmail.com or add me on facebook.
